We are keen to find out what you think of our services and hope you will find the time to give some feedback.

We will use your feedback to try and improve how we work.

You can do this in one of three ways:

Web: Visit www.patientopinion.org.uk and click on the pink ‘Share your story’ box. (You can also see stories from other people that may be of interest). Alternatively, you can ‘Share your story’ below.

Phone: 0800 122 31 35 (free phone) during working hours and tell your story to someone at Patient Opinion.

We’re asking Patient Opinion to hear your story for us because:

• Patient Opinion is entirely confidential. Although we will hear what you say, we don’t know who said it. So you can feel free to say exactly what you want.
• What you say will be shared on the Patient Opinion website. Other patients can see what you thought of the service, and the NHS can use your feedback to improve services. This is done without revealing your contact details.
• If you do not want anyone to know who you are, we advise that you avoid putting identifying details in your feedback.

I do hope you’ll tell us what you thought of the service you experienced. It will help to improve the health care for other patients as well as yourself.
